Instructions for use of the OFA Chow Chow Survey
Below are the Instructions for use of the OFA Chow Chow Survey :
We would like for you to go back only 5 years on the dogs that you own. Dogs that have been in your posession since 2003 to present. Living or deceased.
When you open the survey you will be asked if you have filled out the survey before or if this is the first time. Just click "yes" or "no". Then click continue at the end of this section.
Please also fill out the box that ask for your country. We would like to be able to get a feel of who has what problems in what countries.
You will then be asked the sex of your dog, if the dog is still living and if the dog is spayed or neutered.
Click "continue" at the end of each of these sections.
You will then be presented with a list of "yes" and "no" questions about health issues that may apply to your dog, i.e., cancers, eyes, liver, ears, temperament, teeth, etc.
Click "yes" to the ones that apply and "no" to the ones that do not. At the end of this list click "continue".
The "yes" questions that you checked will then take you to a more indepth section that lists the different types of eye diseases, cancers, temperament problems, etc.
The "no" questions that you checked will allow you to automatically skip those subsections.
Be sure to click "complete" at the end of each of these sections. When you have finished this particular area, you will then go to "breeding status". Again, click "yes" or "no" to these questions.
The survey will then let you know when you have reached the end. Click on "finish" and you are done.
To get a feel for the survey, you may click on the results section to see what the tally is for each of the sections and to see what each of the sections include.
You can only do one dog at a time. But the survey is very fast to go through.
This survey is extremely important for our breed and we are asking all chow owners to participate.
